Monthly Premiums are based on the number of employees in a group. (A group consists of 100% participation per classification of employees with optional Spousal and Children coverage)

How to Apply OPTIONAL SPOUSE: $18.90/Spouse, per month OPTIONAL CHILDREN: $10.00/employee per month OPTIONAL SPOUSE: $14.50/Spouse, per month OPTIONAL CHILDREN: $10.00/employee per month OPTIONAL SPOUSE: $11.60/Spouse, per month OPTIONAL CHILDREN: $10.00/employee per month Note: all DSAI cheque payments should be made payable to: Industrial Alliance Insurance and Financial Services Inc.
